Athletes may pre-register for Healthy Athletes by visiting bit.ly/2018-SONC-SG-Healthy-Athletes**IMPORTANT**: Minor athletes must have their parent or guardian complete this electronic document on their behalf and are encouraged to do so, especially if they are not traveling with their athlete (coaches may not sign for minor athletes if not their parent/guardian). If, instead of completing the online pre-registration, the athlete can download, complete and bring a paper consent form to the games, by visiting bit.ly/2018-Healthy-Athletes-Consent

When athletes arrive at the Healthy Athletes Pavilion they need to check-in at the registration desk before visiting the Healthy Athlete stations. At the registration desk the athletes will receive a wristband indicating they have completed the required paperwork to visit the stations.  At the registation desk the athletes will also receive a passport. When athletes complete a Healthy Athletes station they will get a sticker for their passport and when finished, the athletes can turn in the passport at the check-out table for a prize.
