In order to be eligible to participate on a sports team, you must first complete the online athletic clearance process. All paperwork must be filled out completely and uploaded prior to the beginning of the sports season. Failure to complete any of these forms will make the athlete ineligible.

2) UPLOAD the following 4 documents AS ONE PDF in Step #1 - Student Information - Physical / Additional Forms Uploads

- The signed confirmation page (FOR EACH SPORT) - We will need wet signatures from both athlete & parent. This document can be found under 'VIEW' in and it's emailed directly to you upon completion of the online process. Please include the confirmation page for EACH sport that your child is intending to trying out.

- Completed physical exam form - Physicals are good for one year. Please make sure your physical is up-to-date. If not, please schedule a physical with your doctor.
