Athletic Clearance

Please click on this link to register / complete the online clearance and print/ sign the confirmation page once completed - This will need to be turned in to the athletics office along with the Paperwork due*** (see below)

Fall     - Due June 15
Winter - Due October 
Spring - Due January 15

In order to be eligible to participate on a sports team, you must first complete the online athletic clearance process. All paperwork must be filled out completely and turned in prior to the beginning of the sports season. Failure to complete any of these forms will make the athlete ineligible.

***Paperwork due:

*If the athlete is trying out for a sport for the first time for the 2018-2019 SCHOOL YEAR - turn in:

1) the confirmation page - You will receive an email containing this form once the online athletic clearance process is complete.

2) completed  physical exam form and 

3) front/back copies of your insurance card.

4) Grades from previous semester (if did not attend TPHS).

 5) CIF Pre-Enrollment Affidavit (if did not attend SDUHSD last year).

6) Private Auto Use Form - if parent plans to drive child to and/or from a contest

   *Note: If the athlete participated in a fall and/or winter sport of the same school year - ONLY turn in the confirmation page (received by email) for the season the athlete is trying out in. If the athlete is trying out for more than one sport in a season, a confirmation page is needed for each sport.

   Tuesday, August 28 at 6 PM in the Lecture Hall
   Thursday, September 6 at 6 PM in the Lecture Hall
   Thursday, September 13 at 6 PM in Room 403
Mandatory Fall Student Athlete Meeting - Tuesday, August 27, 2 PM in the Lecture Hall
Mandatory Fall Parent Athlete Meeting - Tuesday, August 21 at 6 PM in the Lecture Hall

All San Dieguito Union High School District student-athletes and their parents/guardians are required by
the district to attend their school site’s preseason orientation meeting. The student athlete will not be able to
participate in their first contest until the student athlete and one parent/guardian has attended the meeting.
