Athletic Clearance

Athletic Clearance Deadlines 2021/2022:
Cheer - due May 19, 2021.
Fall Sports are due July 15, 2021.
Winter Sports are due October 15, 2021.
Spring Sports are due January 15, 2022.

Instructions below. Please read carefully:

In order to be eligible to participate on a sports team, you must first complete the athletic clearance process every new school year. All paperwork must be filled out completely and emailed to or dropped off at the TPHS Administration Building / Athletics Office by the deadline listed above. Failure to complete any of these forms will result in the ineligible to tryout, practice, and/or compete.

Step 1 - Register/login to
  • Create a new clearance and completely fill in all information requested. 
  • This online clearance process must be done for each interested sports.
  • Print and sign Confirmation Page.

Step 2 - Submit the below forms:
  • Signed Confirmation Page
    • See Step 1
  • Physical Exam Form
    • Physicals are good for one year. Know when your physical expires.
    • If your physical expires, you will be immediately uncleared until an updated copy is submitted regardless of previous clearance status.
  • Private Auto Use Form (Important - Read below)
    • This form is for any instance an athlete would need to be driven to/from a sporting event and can not take the bus provided.
    • Athletes are NOT able to drive themselves to/from a sporting event when a bus is supplied.
    • No carpooling under any circumstance when a bus is supplied.
    • A form is required for each parent/guardian driver in your household.
    • You must meet the minimum auto policy limits listed for your form to be accepted.
  • HEALTH Insurance Card
    • Scan or take a clear picture of the front AND back of your card. (Active Military ID is acceptable only if no card is available)

Step 3 - Transfer Paperwork (where applicable)
If you did NOT attend TPHS and/or did not attend a SDUHSD school last semester, please also complete and submit the following:
  • Grades/transcript from previous semester
  • CIF 510 Pre-Enrollment Affidavit
  • If you did not start at TPHS at the beginning of 9th grade, you are considered a transfer student. Please click here for more information.

All SDUHSD student-athletes and their parents/guardians are required by the district to attend their school site’s preseason orientation meeting. The student-athlete will not be able to participate in their first contest until the student athlete and one parent/guardian has attended the meeting. These meetings dates will be released sometime AFTER tryouts and rosters are set. Click the hyperlink above only if you know you did NOT attend the mandatory Parent or Athlete Presentation. It's important to note that if you have already completed this meeting or presentation for a previous sport, you do not need to attend the mandatory meeting or complete the presentation again. This only applies to the Mandatory TP Athletics, please attend all other sport specific meetings.
