Athletic Clearance

Please read instructions below: 

The 2020-2021 Athletic Clearance is now open. With our school site closed and we are unable to collect athletic packets in person, we have made changes to our online clearance process. Please read the following carefully:

In order to be eligible to participate on a sports team, you must first complete the online athletic clearance process. All paperwork must be filled out completely and uploaded prior to the beginning of the sports season. Failure to complete any of these forms will make the athlete ineligible. 

1) Please sign on to www.athleticclearance.com and complete the online clearance process. If you are completing clearances for multiple sports, on the confirmation page, apply the clearance to each sport your child will be trying out for.

2) UPLOAD the following 4 documents AS ONE PDF in athleticclearance.com: Step #1 - Student Information - Physical / Additional Forms Uploads 

- The signed confirmation page (FOR EACH SPORT) - We will need wet signatures from both athlete & parent. This document can be found under 'VIEW' in athleticclearance.com and it's emailed directly to you upon completion of the online process. Please include the confirmation page for EACH sport that your child is intending to trying out.

- Completed physical exam form - Physicals are good for one year. Please make sure your physical is up-to-date. If not, please schedule a physical with your doctor.

- Front/back copies of your insurance card - Please click here if you need to purchase low cost accident insurance from Pacific Educators Insurance Services

- Private Auto Use Form (required for all athletes since transportation is currently unknown)


3) If the athlete participates in a fall and/or winter sport of the same school year,  ONLY upload the signed confirmation page for the sport the athlete is trying out in. A signed confirmation page is needed for each sport.


4) For students who did not attend TPHS and/or did not attend a SDUHSD school last semesterplease also upload the following:

5) If you did not start at TPHS at the beginning of 9th grade, you are considered a transfer student. Please click here for more information.

Mandatory Player / Parent Meetings - (TBD)   

All San Dieguito Union High School District student-athletes and their parents/guardians are required by the district to attend their school site’s preseason orientation meeting. The student-athlete will not be able to participate in their first contest until the student athlete and one parent/guardian has attended the meeting.

Fall Mandatory TP Parent AND Athlete Meetings (Yes, they are together). All meetings are TBD; sport specific meetings will follow the parent and athlete meeting. 
*** If the parent/athlete participated in a fall or winter sport, they do not need to attend but please attend your sports specific meeting that begins at 6:30 PM.
