Please remember to fill out the Athletic Handbook Entirely. This requires signatures from parent/guardians and student/athletes. You may drop these off at Sumner Memorial High School's main office.

Any student participating in co-curricular activities; must have proof of insurance and a physical on file at the school. Physicals exam clearances can be faxed to the school at (207) 422-6463
