FREE/REDUCED LUNCH APPLICATION

The 2021-2022 Free/Reduced lunch application can be downloaded HERE. Please read the Letter to Household for assistance in filling out the form.

In accordance with federal civil rights law and U.S. Department of Agriculture (USDA) civil rights regulations and policies, the USDA, its agencies, offices, and employees, and institutions participating in or administering USDA programs are prohibited from discriminating based on race, color, national origin, sex, disability, age, or reprisal or retaliation for prior civil rights activity in any program or activity conducted or funded by USDA.

Persons with disabilities who require alternative means of communication for program information (e.g. Braille, large print, audiotape, American Sign Language, etc.) should contact the agency (state or local) where they applied for benefits. Individuals who are deaf, hard of hearing, or have speech disabilities may contact USDA through the Federal Relay Service at (800)877-8339. Additionally, program information may be made available in languages other than English.

To file a program complaint of discrimination, complete the USDA Program Discrimination Complaint Form, (AD-3027) found online at http://www.ascr.usda.gov/complaint_filing_cust.html, and at any USDA office, or write a letter addressed to USDA and provide in the letter all of the information requested in the form. To request a copy of the complaint form, call (866)632-9992. Submit your completed form or letter to USDA by:

Child Nutrition Charging Policy

Parents are encouraged to pay in advance for student meals. Payment may be made by cash or check (made payable to Quapaw Public Schools) submitted to the child nutrition director, elementary secretary, or lunch monitor.

If necessary, students may charge up to $10.00 of reimbursable meals on their accounts. Parents will receive notification by phone or email when their child’s account has a negative balance. Students will be served regular meals while giving parents the opportunity to make a payment. However, if a student’s account reaches a negative balance of $20.00, they will be required to pay cash for a regular meal or be served an alternate reimbursable meal at the regular price until payment is made. Alternate meals will consist of a peanut butter and jelly sandwich or a cheese sandwich with fruit and milk.

Both positive and negative lunch balances will carry forward to the next school year. Students beginning the school year with a negative balance in excess of $20.00 will be required to pay cash for a regular meal or be served an alternate meal until their account is paid in full.






Quapaw Public Schools would like to invite community members to serve on the district Wellness Committee. Our goal is to have a committee that represents our district and establishes an action plan for how we might make our school healthier for all students and staff members. The committee will meet during the noon hour four times a year.
