The Project Outline:

Philanjalo has worked in collaboration with the Department of Health (DoH) over the last few years to develop an MDR-TB management programme suitable for ambulant patients living at home. This was borne out of a necessity to cope with the increasing numbers of drug resistant TB patients who require admission of at least 6 months for the intensive phase of treatment. This led to an unprecedented demand for hospital beds causing the health system to be heavily overburdened and stretched. There is also a personal inconvenience for the patient being away from home for this extended period. The model has been developed to support the patient holistically in the community for the full or near full duration of their treatment. Favorable treatment outcomes have been achieved, showing the success of community management of MDRTB even in informal settlements and resource poor areas. 
Philanjalo is assembling a dynamic team of TB experts who will have the opportunity to provide mentorship and training in order to build capacity in at least 16 sites in 8 provinces where the model can be replicated. The project will entail doing a situational analysis at each site evaluating the strengths, weaknesses, recourses and readiness to start a community management program. The team will then design a training and mentoring programme crafted and adapted to the site, equipping them to initiate their own programme. This standardized model will facilitate the further decentralization of MDR-TB care in South Africa.

Project at a Glance:

The goal of the project is to improve quality of care for MDR-TB patients and build capacity of MDR-TB sites to implement sustainable integrated community management of MDR-TB patients. This will be achieved by providing technical assistance and support to decentralised and satellite MDR-TB units in 16 districts, in eight of the nine South African provinces, to enable them to implement community management of patients diagnosed with MDR-TB.

The technical assistance and support will take place in 3 phases:
  • Phase 1: Philanjalo will facilitate a situational analysis at the identified decentralized sites. A checklist and indicator tool evaluating readiness, current infrastructure, human and material resources available as well as strengths and weakness, in their capacity to rollout a community management of MDR-TB program will be completed. Each of the evaluated sites will receive a report with a project implementation plan based on the assessment.
  • Phase 2: Each identified district will be trained on a locally adapted patientcentered model on the management of MDRTB and HIV coinfected patients in the community for the duration of their MDRTB treatment.
  • Phase 3: Conduct post-training support visits at each site to document and monitor progress.
