Uniform Order Form 2019 - 2020 

Notes:
* This Uniform Order Form is available from March 25 - March 27, 2019.
* Please fill in all personal information completely and select the items that you want to order.
* If the item you need is not listed, please contact the uniform committee.
* Please do the payment after receiving the confirmation (invoice) by email once you submit the order.
* Payment can only be done via transfer to CINDY BCA account 0650 7791 85

Please write the invoice number and student's name on the transfer note. eg MICHELLE LEE U-GR8-F-10

* Please print the invoice twice and submit them together with the payment receipt to the committee the latest by April 28, 2019.
* The uniform committee will take one of the invoices and stamp the other one. 
* The stamped invoice will be given to the students/parents for collection of the uniform (the exact date will be informed later on).
