Welcome to PRHS Health Office~ Denise Petrycki, RN BSN

Any Freshmen who will need to have a Sport's Clearance on record.  This requirement can be satisfied in several ways:  1.)  If a Sport's Clearance was completed in the Spring of 2016 at your elementary school or 2.) If you have had a physical completed by your primary care physician dated April 1, 2016 or later.  According to the NHIAA (New Hampshire Interscholastic Athletic Association) you only need one Sport's Clearance during your high school career. PRHS holds 3 dates for Sport's Clearance where Plymouth Pediatrics comes to  PRHS and completes physicals for students wishing to partake in a sport and does not have a current physical. Dates will vary and be posted in Bobcat Beat a month prior to each sport season.

If you are new to SAU #48 and are planning on participating in a Sport then you will need to have a Sport's Clearance on record.  You will need to provide documentation of a physical completed by your primary care physician dated April 1, 2016 or later. You only need one Sport's Clearance/Physical for your high school career. This is for students who have NOT had a physical.

If you have participated in any sport at PRHS and have a Sport's Clearance/Physical on file then you do not need to have another Sport's Clearance/Physical.
