INCIDENT REPORTS

Here are guidelines for when to fill out an incident report

If the incident involves a student with an IEP, you should fill out the special education-specific form, here

If the incident does not involve a student with an IEP (i.e., if it involves a general education student or a non-student), you should fill out the general form, here.

If the incident happens during the after school program, the site coordinator fills out the same form as above. The site coordinator will select that the incident happened after school. They will forward it to the site principal for signature and then the site principal will fwd to the expanded learning team. InformedK12 guides you along the process so you get it right.
