Registration dates for the Fall Season 2016

Saturday May 7th @ Taft Elementary 12:00-2:00 pm

Saturday May 28th @ Taft Elementary 10:00am-12:00 pm Cancelled

Friday June 10th @ Steve Ambriz Park, 611 Riverbend Parkway, Orange CA  6:30-8:00 pm

Saturday June 25th @ Taft Elementary 9:00am-12:00pm

Saturday July 16th -Location & Time TBD

-July 16th will be the LAST registration for Fall Season 2016

 

Please complete the following to register your child for the Fall Season 

1. Go to eayso.org

2. Fill out a Returning/New Player Application form

3. Fill out a Returning/New Adult Volunteer form

4. Print out 2 copies of the Returning/New Player Application form AND 1 copy of the Returning/New Adult Volunteer form

5. Come to an in-person registration date and bring:

-the 3 forms mentioned above

-Proof of residency (a copy of a utility bill, credit card or bank statement, tax or escrow papers, or car registration, dated within the last 6 months)

-Proof of age for the player

-Picture ID for the volunteering adult

-Payment, $125.00 early registration (until June 10th.) After June 10th., fee goes to $145.00 in the form of Check, Cash, or Credit Card.

 

**ALL documents listed above MUST be submitted, on registration day, for registration to be complete. Players will not be placed on a team until registration requirements are complete. **
