Required Protocol Forms: Can be found online at the NC Science and Engineering Fair website and printed out for signatures. All approval forms must be signed and dated before experimentation begins. Any missing forms can result in disqualification.

Form 1B – Approval Form
Form3-Risk Assessment Form-Required by NHCS for ALL projects
Research Plan Attachment (This should be a typed one page summary of the project with references)

Higher risk projects require additional paperwork.

Extremely Important Rules to Follow:
Pathogen Studies may NOT be conducted at home. Baker’s and Brewer’s yeast are not considered pathogens. Elementary students are NOT allowed to do bacteria projects!
Students doing studies with vertebrates must have the supervision and signatures of appropriate scientists, veterinarians, etc. All projects must be approved by the SRC committee before research begins.
The use of photographs of experiments is encouraged! Acknowledge the photographer on the presentation board. The use of students other than the student researcher in photographs is discouraged.
The school’s IRB Committee must review any research involving humans BEFORE experimentation begins.
Form 4-Human Consent Form must be completed for all projects using human subjects when more than minimal risk is determined by the school’s IRB Committee. This does include surveys and questionnaires.
