Portuguese-speaking people often use two surnames: the first is the mother's family name, the second the father's, sometimes joined by y (and).

It is never incorrect to give both, but often one is dropped.  Usage varies within families; follow the individual's preference if this can be learned.  If not, follow these general guidelines:

            Senhor José Almeida Rebouças                        His wife: Senhora Rebouças
            Senhor Rebouças (not Senhor Almeida)
