Minor Behavior Tracking Form

A minor behavior tracking form is filled out as a way to track student behavior as well as staff interventions. When you fill out this form it would be a Level 3 situation.

Process for Minor Referral:

1) Complete the Minor Behavior Tracking Form. This is an internal document that is not meant to be seen by families or students. This form is meant to provide data of chronic minor misbehaviors and provide opportunities for interventions and/or coaching.

• 6th Grade - Minor Behavior Tracking Form
• 7th Grade - Minor Behavior Tracking Form
• 8th Grade - Minor Behavior Tracking Form

2) During Team PLC's, check the Minor Behavior Tracking Form Data to assess patterns and trends as well as discuss interventions.

3) If necessary, make an appointment with the Student Management Teams (SMT) to discuss.
