Major Referrals

A major referral is filled out when a student is at a Level 4 or 5 situation.

Process for writing a major referral:

Level 4 behaviors: At this level multiple interventions (examples below) have not worked yet.

1) A class expectation reminder/discussion

2) An individual expectation reminder (this could be in the moment, after class or while students are working)

3) Modify the classroom environment to help the student be successful (ie seat change)

4) Send student to the buddy class with a reflection/think sheet (max 10min)

5) Revoke privileges (a classroom consequence)

6) Student contacts parent/guardian contact with staff member present and also talking

7)Written plan for behavior improvement (connect with student's counselor)

8) Parent/Guardian meeting/conference

• Download the NCSD referral form and save it to your desktop.
• Open the saved NCSD referral form from your desktop, fill it out, and "save as" the student's name.
• Email the form to our behavior specialist - Lorena Jasis-Wallace (jasiswallacel@nclack.k12.or.us), CC' Karly Lefferts, and keep a copy for yourself.
• Staff member who wrote the referral will make parent/guardian contact about the incident.
• The Student Management Team will process the referral.
• The Student Management Team will meet with the student and debrief the incident.
• The Student Management Team will communicate with student's family.
• The Student Management Team will check-in with staff member who wrote the referral.
