IMPORTANT REMINDERS
• NEW VACCINE REQUIRED FOR 7TH GRADE STUDENTS: 

Parents of grade 7 students: Maine Law now requires Tetanus/Diphtheria/Pertussis (Tdap):  Effective for the start of school year 2017, 1 dose of Tdap vaccine is required for children entering 7th grade.

SECTION 504 PLANS: Students with a physical or mental impairment that substantially limits a major life activity may be eligible for Section 504 accommodations or services. If you have questions about whether your child may be eligible for such services, please contact the GMS School Nurse or School Counselor.






MEDICATION AT SCHOOL
If your child has medication(s) with the nurse at school, they MUST be accompanied by a completed Medical Consent Form (below). Controlled medications MUST be delivered by an adult to a school secretary or nurse. Children/students may not transport controlled medications to or from school. All meds must be picked up on or before the end of school. Medications left during the summer will be discarded.
