Diabetes Health Plans
SECTION 504 PLANS: Students with a physical or mental impairment that substantially limits a major life activity may be eligible for Section 504 accommodations or services. If you have questions about whether your child may be eligible for such services, please contact the GMS School Nurse or School Counselor.
PREPARE FOR FLU SEASON: Please do not forget your child’s flu shot. Make appointments with your provider as early as possible to get the most benefit from your yearly vaccine. For more information about seasonal flu and how to protect your child: Flu Information
MEDICATION AT SCHOOLIf your child has medication(s) with the nurse at school, they MUST be accompanied by a completed Medical Consent Form (below). Controlled medications MUST be delivered by an adult to a school secretary or nurse. Children/students may not transport controlled medications to or from school. All meds must be picked up on or before the end of school. Medications left during the summer will be discarded.
