Google Forms is an online form submission program, but the capabilities and uses for this type of program are limitless.

Creating a Google Form:

Open your “Waffle” and click on the Google Drive.

Click on the red “New” button, and navigate to Google Forms.

Options on the Form:

The first part of the form are the submission options. You can do things such as require a milfordma.com login to access the form, only allow a single submission per user, shuffle question order, and more.

There are additional options at the end of the form. You can allow users to go back and edit their submissions after they turn them in.

Initial Setup of the Form:

Give your Form a title.

Add a description if needed.

Adding Questions:

There are several types of questions you can insert, and each will provide you with a different type of response. Use these screenshots to guide you in choosing and setting up the types of questions you need.

Type in the test of your question.

Add any Help Test needed (additional directions, hints, etc)

Choose your question type.

Check off if the question is required.

Duplicate the question if you have more of the same type of question, or add a new question.

Text/Paragraph Text

The user will type in their own answer. Use the Paragraph Text if you are expecting anything longer than a few words.

Multiple Choice/Checkbox/Choose from a list

All three questions are created the same way, but display differently. The MC allows for only one choice of a few options. Checkbox allows for multiple choices in the same answer. List is best when choosing from a larger list, such as what state a person is from.

Scale

This question has the user rate a given criteria using a unique scale (1-5, 1-10) with labels (high-low, agree-disagree, etc)

Grid

This type of question allows the user to measure multiple criteria using the same scale - similar to a behavior/psychological evaluation form or rubric.

Date/Time

This type of answer requires the user to input a specific date or time as their answer.

Changing the Theme:

You can change the background and header image of your form to suit your purpose. Click on the button “Change Theme” and choose a theme from the side menu. You can then change the header image by clicking “CUstomize” on the theme you choose. You can select from any of the Google images, or upload your own.

Sending the Form:

You can send the form out by sending a URL. Simply copy the URL from the box and paste it into an email or into your internet browser address bar.

You can also send an email directly from this screen. Simply type in the emails or groups you want to send it to, and format your email and click “Send”.

What Your Students Will See:

Here is what your students will see when they login to your form.

And when they submit the form, this is what they see.

Viewing Responses:

As students respond on the form, Google automatically creates a Google Sheets that keeps track of the student responses. Each question is recorded in it’s own column, and you can monitor who has turned in their form, if they made changes, and then turn off the form when everyone has completed it.

Google Forms is a survey administration app that is included in the Google Drive office suite and Google Classroom along with Google Docs, Google Sheets, and Google Slides. It can also be used for assessment.
