Additional Helpful Information
If you are unable to locate your Medicare card, there are three ways you can request a replacement. You can visit your local Social Security office, or you can call Medicare at 1-800-772-1213, or visit them online at www.ssa.gov/medicarecard to request a replacement card.
When you visit a doctor or go to the hospital you will only need to show your Anthem Medicare Advantage ID Card.

When you fill RX at the Pharmacy you will need to show your Anthem Medicare Advantage ID Card.
