The following medical information
and/or requirements are needed for each student enrolled at The Maine Girls' Academy for the 2016-2017 academic year:
Combination Emergency/Participation/Parental Approval Form and
Biographical Form (double sided) to be completed and signed by a parent or guardian. You will need
to complete this form at the beginning of each school year. Students
will not be allowed to attend classes until this form is completed and on
file with the Main Office.
If your daughter is involved in a club
or a sport, please note that this form may be provided to the adult in
charge to have on file in case of an emergency.
Medication Authorization Form must be completed and signed by a
parent or guardian. No student
will be given medication unless this form is returned to the Health
Office. Parents and Guardians must complete this form at the beginning of each school year.
Physical: A physical is required
of all first-year students. Transfer students need to show proof of
having had a physical within the past two years. If the appointment for
the physical cannot be made before the start of the school year, please
notify the Health Office at 207-797-3802 x2045 as to the date scheduled.
ImPACT Test: Immediate Post Concussion Assessment and Cognitive Testing. If your daughter is involved in a sport, or wishes to be tested, she is required to take the computerized exam that assists health care providers in the diagnosis and managements of concussions.
Vision Screening: In following State of
Maine guidelines for Vision Screening, all 9th graders will be
screened for distance acuity within the first quarter of the school
year. Parents will be notified of
any issues found with their student’s distance vision screening, which
will include a written recommendation for a follow up with your own
physician. This screening is performed in the Health Office by the school
nurse at no charge to the family
Immunizations: In order to ensure that your
daughter’s immunization information is up to date in her health record, please have the doctor’s office fax or
mail, to my attention, a record of any immunizations she has received
since Kindergarten registration. The School fax number is 207-797-3804.
Please note that a copy of your daughter’s immunization records can be
provided to us by a parent.
Please note: Although not required by the State of Maine,
the increase in cases of Hepatitis
B has led health professionals to recommend that you contact
your family physician regarding inoculation of the HBV vaccine. If your
daughter has received the series of Hepatitis B immunizations, please have the
doctor’s office fax or mail a copy of the immunization dates to my attention (fax: 207-797-3804).
All paperwork must be
returned to school by August 8, 2016.
