From Our Nurse - Summer/Fall 2017


Please be reminded as you plan ahead for September that all prescription medication orders must be renewed at the beginning of each school year. Students may not carry any medications, including non-aspirin pain relievers and inhalers, without a doctor’s order.  If a student is permitted to carry or self administer any medication, the doctor must clearly state that in the orders.   Please see the medication forms in the Health Services tab on the district website. Please return the forms to the Health Office by the first day of school.

Any student who has medications, including Epipen auto injectors, in the Health office must pick them up by June 20.  Any medication that is left in the Nurse’s Office will be destroyed by June 23.


Important Reminder:  All students will receive an LPS Emergency Information Sheet on the first day of school. It is imperative that this contact sheet is completed and signed by the parent and returned within the first few days of school.  If you would like your student to receive non-aspirin pain reliever, the REQUIRED parent signature is also on this form. Please do not hesitate to contact me directly with confidential health concerns at 781 334 7213.


A current physical exam is required for entry into tenth grade or once during the high school years.  Health forms are being sent this month to those students who do not have a current physical exam on file.  Updated health forms must be submitted to the School Nurse by August 27th.  Please contact Mary Homan, School Nurse with any questions.


Important reminder for student athletes:


MIAA rules clearly state that any student who plans on playing a sport at the high school must have a current physical on file (completed within the year or thirteen months).  NO student will be allowed to participate without a current physical. Please contact Mrs. Homan at 718-334-7213 if there is a question regarding the current physical exam date on record in the health office.  


Please note: the school district no longer provides sports physicals with the school physician; therefore it is important to contact your treating physician and arrange for an annual physical if you plan to participate in any of the sports programs. Information regarding the start date for fall sports is available through the Athletic Office.


Congratulations to the Class of 2018

A copy of the graduating student’s immunization record and most recent physical is included with the transcripts  distributed with the caps and gowns.  Any student who is a member of the Class of 2016

.may claim his/her health record up to December 2023. All remaining records at that time will be destroyed. Please be sure to place your students health record in a safe place as a record of immunizations are required by colleges and some work places.
