Here are all the forms you will need to complete to allow your child to participate in LB Athletics during the 2023-2024 school year

2023-2024 Physical Information
Sports physicals will be offered at NWO Express Clinic. The Express Clinic is the facility where athletes are referred for imaging and orthopedic consult when injury occurs during evening and weekend hours.
Physicals will be offered May 1st through August 1st at a cost of $25 for our contracted school athletes. Our NWO Express Clinic is located at 15054 E US Route 224 in Findlay and will be accepting scheduled appointments as well as walk-in appointments.
NWO will continue to donate a portion of the collections back to the Liberty Benton athletic boosters based on the number of student athletes that are seen at NWO for physicals. I have linked a document that will be necessary for the student athletes to print off and provide at the time of their appointment in order for us to track the proceeds that go back to Liberty Benton athletic boosters.

Blank physical forms can be found at the link above and hard copies are available in the HS & MS office. You must use this form when getting your new physical.
