Schedule Pick-Up Proxy Form

"If parents or guardian is unable to attend schedule pick up on June 29 - 30, please use the "Proxy" form for person you designate to pick up schedule and pay for all fees and dues.  Designated person must bring their ID along with the form on the day of schedule pick up."


2017-2018 Schedule Pick-Up Days

Schedule Pick-Up Days for all Tracks are:

Thursday, June 29th and Friday, June 30th

9:00 a.m. - 12:00 p.m.

In the Kapolei Middle School Cultural Center (Cafeteria/Stage)

On these days you may pick-up your child's class schedule, ID, planner and pay dues/fees.


Separate checks will be accepted for the following:

  • Lunch Pre-Payment (suggested amount $25.00) payable to Kapolei Middle School
  • Student Government Dues - $15.00 payable to Kapolei Middle School
  • School Bus (prices in application booklet) payable to Department of Education
  • Yearbook - $25.00 payable to Lifetouch
  • Success Kit (basic school supply kit) - $35.00 payable to Kapolei Middle School Aloha Club
  • School and P.E. Uniforms must be paid in CASH only

If you want to avoid the lines at Schedule Pick-Up Days, the office is now accepting payments for school bus, meal account and student government dues.  

School office hours are 7:30 a.m. - 4:00 p.m.  Closed on state holidays.

SY 2017-2018 School Bus Information





Submit Form CS-101 along with the school bus application (Form ST-70) when requesting service due to the following circumstances:
  • Student Resides less than 1.5 miles from the school
  • Student is enrolled on a Geographic Exception
  • Student desires transportation to /from location other than residence
Requests submitted with Form CS-101 will be reviewed and approved/disapproved by the Leeward District Transportation Officer.  Please note that it may take up to 4 weeks for a determination.  Your child will NOT be permitted to ride the bus until your application has been approved.  Parents must make contingency plans during the interim.



Start Dates for the 2017-2018 School Year

First day of school for Red, Blue and Yellow Tracks is Monday, July 10, 2017.

First day of school for 
Green Track is Friday, July 28, 2017.

Important School Information

School Uniforms

School uniform store is closed for the remainder of the school year.

A limited supply of uniforms are available at the office.  Sizes adult small - adult extra large.

The school uniform vendor will be on campus for schedule pick-up days on June 29th and 30th in the cafeteria from 9 a.m. - 12:00 p.m.
