PSAT EXAM REGISTRATION

Students in Grade 10 and 11 have the opportunity to take the PSAT Exam in school on Saturday, October 13, 2018, at Hopedale High School beginning at approximately 7:30 am. This exam is 2 hours and 45 minutes in length and tests students' skills in Evidence-Based Reading, Writing, and Math. Students must register in the Guidance Office beginning on September 4th, and the cost of the exam is $25 (cash or check made payable to "Hopedale Jr./Sr. High School"). Students with documented disabilities (on an IEP or 504 Plan) that allow for extended time on tests may be eligible for extended time on this exam. They must complete an eligibility form well in advance of the test date. Your child should discuss eligibility with their Guidance Counselor as soon as possible.

CONCUSSIONS (Please visit the "Concussions" tab for more information)

A concussion is a minor, traumatic brain injury that may occur when the head hits an object or a moving object strikes the head. It can affect how the brain works for a while. A concussion can lead to a bad headache, changes in alertness, or loss of consciousness.

If you believe your child may have a concussion, whether sports-related or otherwise, Hopedale Public Schools requires the attached forms be completed and returned to the school nurse as soon as possible (Report of Head Injury, Concussion Education Plan Referral). The nurse will communicate with the Guidance Office and Athletic Director (if the student is an athlete). If a physician diagnoses a concussion, your child may be eligible for curriculum accommodations as they recover.

A follow-up appointment is required when the student has been symptom free for a period of time. Please bring the attached "Post-Concussion/Head Injury Medical Clearance Form" to be completed by your physician and return this form to the school nurse. Hopedale Concussion Forms.pdf

Need a transcript? A request for a transcript requires a signed release from the former student (if 18 years of age or older) or from the parent/legal guardian (if under 18 years of age). Please download the attached form below and return to the Guidance Department (address on form). Requests will be processed in a timely manner.

Current Seniors: All transcript requests must go through NAVIANCE. You DO NOT need to complete this form.
