Physical Packet

Dear Parents/Guardians:

Although it has been determined at this time that High Tech High School will not have an interscholastic sports program for the 2017/2018 school year, I am enclosing the physical form that is necessary to be completed for any student that is planning to participate or tryout for a sport at their sending school. Also enclosed is the Sudden Cardiac Death brochure, a concussion information sheet and an eye injury information sheet that is required by the State to be distributed to all students that will be participating. Please include the signed and completed Sudden Cardiac Sign off sheet and the concussion information sheet to be handed in with your child's physical.

Intramural sports will again be offered for the 2017/2018 school year. The following forms: physicalcardiacconcussion must be completed and handed in if your child plans to participate this school year. A physical exam that is less than 1 year old is required for any student that wishes to participate in any intramural sport or dance.  I recommend that any student that might be interested in participating in any intramural sport during the school year should have a physical completed by their doctor over the summer months and hand it in to the Nurse's Office on the first day of school.  This requirement is mandated by the N.J. Department of Education and the New Jersey State Interscholastic Athletic Association (NJSIAA).

The physical examination must be done by your own physician or health care provider.  The enclosed form is required by the N.J. Department of Education and must be utilized by your physician. Please read the attached page regarding the Scholastic Student Athlete Safety ActThis is a new law in the State of New Jersey that requires all physicians that conduct a sports physical to complete a cardiac training module. If your child's doctor does not sign on the physical form that they have completed this training, we will not be able to allow your child to participate in the above programs. The form must be filled out completely by the parent/guardian and the examining physician or health care provider as directed.  This means that every section of the form must be filled out.  The student will not be allowed to try out or participate in the sport unless the form is complete.
