Physicals: All students are required to have a complete PIAA physical form completed prior to any participation in practice or competitions can occur. Physical forms can be picked up at either the MS or HS front offices or by printing the attached physical form below. For any questions please contact Mr. Bob Spertzel, LAT, or Jeremy Flores, AD.
Re-certification -Section 7: All students that wish to participate in a Winter or Spring sport that previously participated in a sport in the fall or winter are required to submit a re-certification form. Section 7 of the PIAA Physical packet is attached below.
Code of Conduct: All Hanover Public School District Athletes are required to follow the Athletic Code of Conduct. A signed contract must be turned into the athletic department before any athletic participation will be permitted. Please see the Hanover Athletic Code of Conduct attached below. In an effort to conserve paper the signature sheet is attached separately.
Physical and Athletics Consent Form: All Hanover Public School District Athletes and Parents are required to have a signed physical participation and/or athletics participation consent form on file with the Athletic Director each school year. Please see the Physical and Athletics Consent Form attached below.
Transportation Release Form: All Hanover Public School District Athletes are required to travel with the team to and from any away athletic events. Exceptions; however, may be granted by the Athletic Director. Please print and turn in the transportation release form below to the Athletic Director no later than 24 hours prior to an away event. No alternate transportation will be granted if that deadline is not met. In addition no alternate transportation will be allowed during any post-season competition except in the case of an emergency. Please keep these trips to a minimum as we subscribe to the following philosophy with our athletes. "We travel as a team, compete as a team, and win/lose as a team."
