Welcome to the Online Clearance Process Guide. Here you will find the directions to complete the clearance process to participate in athletics and activities; please review both portions of the clearance process.  A PARENT must create an account at AthleticClearance.com to clear their student athletes. You will need to create a "new clearance" for each sport you participate in, every year. Do not print out the consent form from a previous season as you will NOT be cleared without entering "new clearance" information, even if you have already participated in that sport in a prior season. 
Online Clearance Portion

Online clearance must be completed for each sport you intend to participate in this year.

Complete the Log-in Process. A PARENT must create an account to clear student athletes.
Step 3
Complete the Online Forms. Always start a new clearance for each season by clicking "Start Clearance Here".  
Step 4
Complete the Online Signature Agreements.
Step 5
Print and Sign the Confirmation/Consent Form. Print from the site or from the email that is sent to your registered email.
Step 6
Bring the signed form (or email) and a copy of a current physical to the Assistant Principal's Office.* 

* - If purchasing Student Insurance, please turn in a copy of the printed ID card to Assistant Principal's Office.

Physical Form Portion

Physicals are required annually and must be valid throughout the entire season of your sport.

Complete and sign the top portion prior to doctor visit.
Step 3
Have licensed health care provider complete the bottom portion of the form. The form can not be completed by a Chiropractor and must be completed in California. 
**NOTE** Kaiser does not use our physical form for their exams. The completed form they provide contains the same information and WILL be accepted by the office if attached to the competed and signed top portion of the school's physical form.

Bring a copy of current physical and the signed clearance/consent email to the Assistant Principal's Office.
