West Hills High School Athletic Clearance Process

Welcome to the Online Clearance Process Guide. Here you will find the directions to complete the clearance process to participate in athletics and activities; please review both portions of the clearance process.  A PARENT must create an account at AthleticClearance.com to clear their student athletes. You will need to create a "new clearance" for each sport you participate in, every year. Do not print out the consent form from a previous season as you will NOT be cleared without entering "new clearance" information, even if you have already participated in that sport in a prior season. 

Please review the West Hills Athletics Handbook prior to clearance/participation in your sport(s) for information pertaining to Eligibility, Behavior Expectations, etc.

STEP ONE: Online Clearance

Online clearance must be completed for each sport you intend to participate in this year. A separate consent form/confirmation message must be signed by the parent for each sport.

1.  Go to athleticclearance.com - CLICK HERE

2.  Complete the Log-in Process. A PARENT must create an account to clear student athletes.

3. Complete the Online forms. Always start a new clearance for each season by clicking "Start Clearance Here" and selecting the correct school year for the clearance. We are currently in the 24-25 school year.

4. Print and Sign the confirmation/consent form. Print from the site or from the email that is sent to your registered email.

5. Bring the signed form and a copy of a current physical (if needed) to the Assistant Principal's Secretaries' Office.

* -Medical insurance is required for all athletes to clear for their sport(s). If purchasing Student Insurance, please turn in a copy of the printed ID card to Assistant Principal's Secretaries' Office.

STEP TWO: Physical Form

Physicals are required annually (expire one year from the date of physical exam) and must be valid throughout the entire season of your sport - if your physical expires mid-season, you will no longer be cleared to participate until an updated physical is received in the office (DO NOT GIVE TO THE COACH DIRECTLY).  The physical must be completed by an MD or DO licensed in the state of California.

NOTE:
*We will accept physicals completed by a Chiropractor only if they are a Doctor of Osteopathic Medicine (DO). We will accept physicals completed by a  Physician's Assistant (PA) or Nurse Practitioner (NP) only if the form is also signed or stamped by a MD or DO

Please have the facility stamp the form with valid contact information and the MD or DO's name.


STEP THREE: Complete Your Clearance

Bring a copy of current physical (if submitting a new physical), and the signed clearance "Confirmation Message" to the Assistant Principals' Secretaries' Office. You will need to return to the office to pick up the completed form so you can give it to your coach at tryouts/practice.

NOTE: All clearance forms are reviewed by WHHS Staff prior to approval. Students will be required to clear for each sport individually. You can maintain multiple students with the same parent account.
