To report your child absent or tardy, please call 860-652-7010. For homework, please refer to Google Classroom or the teacher's website.
The State of Connecticut requires that a physical exam be conducted in either 5th or 6th grade, prior to students entering the 7th grade. Students not in compliance will be excluded from starting 7th grade until the information is provided. Completed forms should be submitted to the Smith Middle School Health Office. In addition, state law requires that a physical exam be performed during freshman or sophomore year of high school. The physical must be completed between July 1st after the student's 8th grade year and June 30th at the end of the student's 10th grade year. Any student who is not in compliance with this state mandate will not receive their schedule over the summer for the junior school year. You and your physician must complete the Connecticut State Health Assessment form and submit it to the GHS Health Office. Forms may be faxed to 860-652-7274, mailed to GHS Health Office, 330 Hubbard Street, Glastonbury, CT 06033, or dropped off by the student or parent. The form is available in the left sidebar. Please note: Sports physicals DO NOT satisfy this requirement. Contact the GHS Athletic Department to access their requirements and forms.
Students are not permitted to carry any medication in school without proper authorization.This includes Tylenol, Advil or any over-the-counter medication. State law and the Glastonbury Board of Education requires an Authorization for the Administration of Medication by School Personnel Form be completed by a Doctor, Dentist, APRN or Physician's Assistant. The form must also be signed by a parent or guardian. Medication must be brought to the health office in the original labeled container. ALL medication must be kept in the health office; Students may not carry medication except in certain emergency situations. Epi Pens and Asthma Inhalers may be self administered and self carried if it is indicated on the Medication Administration Form. A new form is required every school year. Controlled substances must be brought in to the health office by a parent. Please download the form from the left sidebar. Please call us with any questions.
Screenings will be conducted in the health office according to our district policy. A referral will be sent home if any student does not pass the screening. If you do not hear from the school nurse, assume your child has passed. If you would like more information, feel free to contact the nurse.
