Online Substitute Teacher Evaluation

In an ongoing effort to improve the educational experience of our students, District 219 is requesting that teachers provide feedback on substitute teachers. This will allow quick and easy compilation of feeedback and we hope you will also find it more efficient for entering evaluations. After a substitute teacher has covered a class, you will receive an email containing a substitute teacher evaluation form. Please complete and submit the form in a timely manner. Instructions follow.

The teacher will receive an email notification which will include a link to the evaluation form.

When using the link from this email, the form will open with some fields pre-populated.

Complete the remaining fields and submit. If you would like to receive a copy of your responses, be sure to check the Send me a copy of my responses box.
