Dear District 128 Parents/Guardians:
The Illinois 5Essentials Survey is open for participation now through January 16, 2017. To begin the survey, click HERE.
Teachers, parents and students across Illinois will have an opportunity to participate in the Illinois 5Essentials Survey: Organizing Schools for Improvement. District 128 participated in the 2012-13 and 2014-15 administrations of the survey. To meet state legislation and the ISBE mandate requiring participation every other year, D128 will administer the survey again in 2016-17. As a parent, you will have the opportunity to share your thoughts on the important elements of school effectiveness in a survey about your child’s school. Students will participate in the student version of the survey during the school day before January 16, 2017. Please review the Student Participation and Opt Out information by clicking here. The information includes a form that must be returned to the school's main office by December 5 if you do not wish your child to participate. The Illinois 5Essentials Survey is administered online by UChicago Impact at the University of Chicago, on behalf of the Illinois State Board of Education (ISBE). This survey provides a comprehensive picture of a school's organizational culture in an individualized report measuring five "essentials" critical for school success:
· Effective Leaders
· Collaborative Teachers
· Involved Families
· Supportive Environments
· Ambitious Instruction
Research has shown that schools that are strong on these indicators are more likely to make gains in student achievement.
School-level reports will be generated if at least 20 percent of parents respond to the survey. The reports will be sent to schools and districts in May 2017. Student and teacher data will be included in each school report card, released by ISBE at the end of October, 2017. 5Essentials Reports for your school will also then be publicly available via the UChicago Impact website. 5Essentials Reports will display summary information, such as the percent of parents agreeing that they feel comfortable raising concerns to their child’s teacher. Parent and student identities will be kept completely confidential and responses will not be linked to any individual.
Your participation in the Illinois 5Essentials Survey will help us understand the conditions at your child’s school and guide improvement. Your identity and survey responses will be kept completely confidential and will never be connected to you or your child.
