CRT Intake Coordinator/Clinician

Full time (37.5)

Master’s Degree

Work as an integral member of teams providing outpatient mental health supports to adults in the Rutland County area.  Duties include diagnostic evaluations to determine eligibility, treatment needs, and program assignments.  Position requires frequent consultation with personnel from the Vermont Department of Mental Health and Vermont’s Designated Hospital system. Intake Coordinator also provides short term services for 7-10 persons enrolled in the CRT program, and individual/group clinical supervision for CRT Case Managers.  Master’s Degree, (MSW, LMHC or Psychologists), and three years relevant experience in Human services are required.

Community Recovery Specialists

Our team provides individual and group based services focusing on developing and maintaining independent living skills, coping skills and social skills.  The successful candidate will need to have excellent written and verbal communication skills, the ability to work both independently and as part of a team, and the ability to work a 10 hour shift.  You must be able to pass a background check and have reliable transportation. A valid driver’s license and the ability to work evenings and weekends is required.

This program is designed to provide a nurturing environment that supports individuals' efforts to reintegrate into their community of residence. Work as an integral member of a recovery oriented, multidisciplinary team providing medical services, education and facility/community based supports to residents while implementing their individual plans of care.  Excellent communication skills, flexibility, and the ability to work as part of a treatment team in a stabilizing environment are required. Candidate must have LPN licensure. Experience working with individuals who experience episodic, acute psychiatric symptoms preferred.

            

Licensed Nursing Assistant

Unique opportunity to provide nursing support to adults experiencing symptoms of a severe and persistent  mental illness in a new short-term, four bed residential program. This facility is designed to provide a nurturing environment that supports individuals’ efforts to reintegrate into their community of residence.   The LNA works under the direction of the Residential Nursing Staff Supervisor as part of a multidisciplinary team of treatment providers, providing medical services, education and personal care to residents while implementing their individual plan of care.  Excellent communication skills, flexibility, and the ability to work as part of a treatment team in a stabilizing environment are required. Must have LNA licensure with experience working with individuals with acute psychiatric symptoms preferred

CRISIS STABILIZATION PROGRAM

 

Crisis Recovery Specialist

Provide support services to adults who experience acute psychiatric episodes in a short-term, 4 bed crisis program. Supervise and help stabilize crises of clients by creating a daily routine to include: problem-solving strategies, coping skills and practice for managing daily activities safely for reintegration into their community upon discharge.  Create individual treatment plans and assist individuals to accomplish specified tasks to reach their goal.  Facilitate small group sessions to provide education and therapeutic support to enhance clients’ abilities to cope with their mental health symptoms.  Admit and discharge clients, coordinating with their treatment team as necessary.  Meal planning and preparation and some light housekeeping are required. Excellent communication skills, flexibility, and the ability to work well in a crisis stabilization environment are required. Human service experience preferred.

CHILD AND FAMILY SERVICES

Child and Family Services provides assessment, referral, individual and group treatment, medication management, crisis intervention, and substance abuse services to children ages birth to 18 years old.  These services are provided in a variety of settings including clinic based services, home based services, community based interventions, and school based services.

 

Child and Family Mental Health/Substance Abuse Clinician

Master’s degree required

Adolescent Substance Abuse and Child Mental Health Program is seeking an experienced (at least 2 years) Master's Level Mental Health Clinician who has a strong background in substance abuse theory and practice. The ideal candidate is licensed or licensable in Mental Health and will have an LADC or be willing to seek licensing in the substance abuse field as well.  Position will provide comprehensive assessments and group/individual/family counseling for an adolescent substance abuse population as well as a mental health population of children ages 6 - 18.

Candidate will be able to provide co-occurring treatment for substance abuse and mental health with an understanding of Motivational Enhancement Therapy and Cognitive Behavioral Interventions.

Therapeutic Case Manager

Provide Mental Health case management/case coordination and skills building support to children ages 6 to 18 and their families.  Individual will need to actively work with school systems and other service providers. Master’s level clinicians comfortable and skilled with individual, group and family treatment for children ages 6 through adolescence.  Position will focus on conducting assessments, providing outreach and crisis stabilization services.  Strong assessment skills needed (and ability to work independently, flexibly, in a crisis driven mode).  Must have knowledge and experience with family centered, strengths based treatment.  Must be comfortable working in both a clinic-based setting and a community/home based setting.  Looking for an organized, team focused practitioners.

School Based Clinician

School Based Clinician that is comfortable and skilled with a diverse range of school-aged children.  Family-centered, strength-based clinical practice with strong crisis intervention and assessment skills needed.  Position requires active participation on an interdisciplinary treatment team.  We are looking for an organized, team-focused practitioner.  License preferred.  Position available for the 2014-2015 school year. 

 

Intensive Family Based Services Clinician

Responsible for providing in-home, family based clinical and support services to families with children in risk of out of home placement or in need of reunification.  The clinician will work closely with other agencies to coordinate services.  Experience working with children, parents and/or families required.  License preferred.

Community Skills Worker

Person able to work intensively one on one with children in the community or with families to teach them skills.  Case load of 1 to 2 children at a time for intensive short term services.  Flexible hours (some early evening and weekend availability) that may vary from child to child.  Good team player and communicator.  Reliable transportation needed.  Experience with children preferred.

Tobacco Prevention Coordinator

Part time (15 hours), Temporary (until June 30, 2015)

Bachelor’s degree preferred

Position requires an understanding of policy development and a desire to advance current tobacco prevention efforts.  Skills in advocacy, public speaking, and working with youth are needed.  Bachelor’s degree in community health, education, human services, nursing or related field preferred.  Applicants must be tobacco-free for at least one consistent year prior to applying. This position will mostly occur in a professional office setting along with some community work, and is a daytime, weekday position.
