Seeking an individual to join our team of professionals as the Chief Financial Officer.  The preferred candidate will have a minimum of 5-7 years of financial experience working in the healthcare field including overseeing finance, budgets and billing.  We are looking for a positive, team-oriented individual who is pro-active and results-driven, who is a strategic thinker and a creative problem solver.  Prior experience or project management skills related to technology updates in the area of electronic medical records and health care applications preferred.

Residential Recovery Coordinator (1 Open Position)

Bachelor’s degree preferred, RN considered

Together, this leadership team ensures that operations of two, 4-bed residential treatment facilities on a shared campus conform to meet State and Federal regulations with regard to their specific programs.  Included with this is the oversight of staff, staff training and coordination of services between residential staff and Clinicians.  Seeking someone who sees themselves as a collaborator between clients, treatment teams and community providers.  Must have strong computer skills and be able to work with electronic medical records.

 

Nursing Staff Supervisor

Full time (40 hours)

Unique opportunity to oversee see a team of nurses (RN, LPN and LNA) assigned to work with adults who experience acute psychiatric episodes in one of several residential programs. Nursing staff are integral members of a recovery oriented, multidisciplinary team providing medical services, education and facility/community based supports to residents while implementing their individual plans of care.  Excellent communication skills, flexibility, and the ability to work as part of a treatment team in a stabilizing environment are required. Candidate must have LPN licensure. Experience working with individuals who experience episodic, acute psychiatric symptoms preferred.

 

Recovery Specialists

Provide support services to adults who experience acute psychiatric episodes in a short-term, four bed residential program.  Assist residents with building skills for mental health stability and independent living including problem-solving strategies, coping skills and managing daily activities for eventual reintegration into their community.  Recovery Specialists will implement individual treatment plans, assist individuals with accomplishing specified tasks to reach their goals, and facilitate small group sessions to provide education and therapeutic support to enhance clients’ abilities to cope with their mental health symptoms.  Excellent communication skills, flexibility, and the ability to work as part of a treatment team in a stabilizing environment are required. Human service experience preferred. (Looking for evening, overnight and weekend coverage)

 

Licensed Nursing Assistant

Unique opportunity to provide nursing support to adults experiencing symptoms of a severe and persistent  mental illness in a new short-term, four bed residential program. This facility is designed to provide a nurturing environment that supports individuals’ efforts to reintegrate into their community.   LNA staff work under the direction of the Residential Nursing Staff Supervisor as part of a multidisciplinary team of treatment providers, providing medical services, education and personal care to residents while implementing their individual plan of care.  Excellent communication skills, flexibility, and the ability to work as part of a treatment team in a stabilizing environment are required. Must have LNA licensure with experience working with individuals with acute psychiatric symptoms preferred.

 

CHILD AND FAMILY SERVICES

Child and Family Services provide assessment, referral, individual and group treatment, medication management, crisis intervention, and substance abuse services to children ages birth to 18 years old.  These services are provided in a variety of settings including clinic based services, home based services, community based interventions, and school based services.

Master’s Degree required

School Based Clinician that is comfortable and skilled working with a diverse range of school-aged children.  Family-centered, strength-based clinical practice with strong crisis intervention and assessment skills needed.  Position requires active participation on an interdisciplinary treatment team.  We are looking for an organized, team-focused practitioner.  License preferred.  One position remains available for the 2015-2016 school year, training starts immediately.

 

Community Based Case Manager

Assist families in accessing services in the community; improve transitions of care for children and families in crisis, and serve as a liaison with area medical practices with the provision of comprehensive coordination of care.  Individual needs to be a team player, organized and detail oriented.  Must be able to transport children in your own vehicle.  Bachelor’s degree required, experience preferred.

 

Behavior Interventionist

Person needed to provide specific, individualized, goal oriented services to assist children with behavioral and emotional challenges in high school and grade school classroom.  Candidates will have experience helping children with serious behavioral challenges to remain in regular classroom setting.  Both positions require superior listening skills along with effective verbal and written communication skills and the ability to quickly form effective relationships.

Person must be able to work intensively one on one with children and their families to assist with hospital diversion and crisis stabilization.  Flexibility with hours required due to need for evening and weekend availability.  Seeking an effective team player/communicator with reliable transportation.  Experience with children’s mental health and the ‘wrap around” model preferred.

 

Respite Worker

Bachelor’s Degree preferred

Work one to one with children in the community providing a mix of recreation and mental health support.  Scheduling based on client needs.  Preferred availability weekends and after school hours.  Experience working with children preferred.  Must have reliable transportation.

RAP COALITION

The Rutland Area Prevention Coalition works with schools, civic and community organizations, businesses, law enforcement, and all interested individuals throughout Rutland County to provide education, information, advocacy, support and resources for the prevention of alcohol, tobacco and other drug abuse.

 

Youth Prevention Coordinator

Part Time (15 hours)

Bachelor’s Degree Preferred (Can be used as Internship for Junior or Senior College students)

Position requires an understanding of policy development and a desire to advance current tobacco prevention efforts.  Skills in advocacy, public speaking, and working with youth are needed.  Transportation necessary to travel throughout Rutland County making presentations.

Empower a Life!

Community Access Program (CAP)

 

Shared Living Providers (SLP) needed!  Shared Living is a model of support where a family or individual opens their home to an individual with a developmental disability. Individuals needing homes and support vary in age from school age to elders. The SLP gives support with day to day activities, including personal care, budgeting, accessing medical care, social skills, and assistance to be a part of their home, family and community. Some individuals require more assistance and provider skills to address behavioral issues due to co-occurring mental illness or autism, among other needs and diagnoses.

 

Shared Living Providers come from all walks of life. There is not one specific type of person/family we seek as Shared Living Providers. Be willing to try something new, have fun, learn things about yourself and those around you and make life better for someone else in your community.

 

Shared Living Providers are paid a stipend under a yearly contract with the Community Access Program, a division of the Community Care Network. Shared Living Providers are independent contractors, not employees of the Community Care Network and are not eligible for benefits offered to employees.
