Please remember once you turn 18 years of age, you have (1) one year to apply for your trust dollars and will be forfeited if you miss the deadline. Please submit clear copies of Identification along with completed Form 1 application.

Parents or guardians or trustees or executors of a 18 year old must complete Form 2 along with clear legible photocopies of  identification of both the 18 year and yourself, along with court documents stating you are the person looking after the 18 year old financial affairs. You only have (1) one year to apply after the 18 year old turns of age.
Form 3 is to be used for a deceased loved one, with the same requirements and along with a death certificate.

Please note** according to the policy the time has now ended for adults to apply for the Flood Claim Per Capita Distribution. The (3) three year timeline was from May 16, 2013 to May 16, 2016
