Forms and Physicals

All students who wish to tryout must have the following BEFORE they will be permitted to attend (all forms can be downloaded from the school website at: www.asfms.net/athletics/index.htm ).

Forms may be dropped off on Thursday, August 30th in my classroom (room 127 in the middle hallway) or at the beginning of tryouts.  I prefer that they are submitted early in the day, as it helps to organize paperwork ahead of time.

  1. Signed and notarized Assumption of Risk Form

  2. Signed Athlete’s Participation Pledge

  3. Completed Permission to Participate/Physical Exam Form

    • In order to participate in interscholastic athletics, athletes must have a physical that pronounces them “physically fit for athletic participation”. It needs to be dated within the last calendar year and run through the soccer season (mid- November). Students who do not have a current physical will not be allowed to practice.

    • If you cannot visit your own doctor, the school will conduct athletic physicals immediately after school on Thursday, AUGUST 30th (tentatively) in the nurse’s office at ASFMS.

What to bring:

  • Completed forms

  • Shin Guards/Cleats/Size 5 Soccer Ball

  • All students must bring their own water container to tryouts

  • All players should eat a small snack at the end of the school day (PB crackers, banana, etc)

If you have any questions please contact me at: simasalicia@coventryschools.net

Thank You,

Ms. Simas
