Application for Athletic Pass for home events


ATHLETIC PRACTICE PERMITS (Blue Card)
(High School Athletes Only)

All athletes must get a practice permit from the athletic office before they will be allowed to participate in practice or tryouts.
Permits are issued after $41 participation fee is paid and all paperwork has been submitted. Required paperwork includes Acknowledgement of Codes & Policies with Medical Emergency Form and Physical Card or Alternate Year Form. **$41 fee must be submitted every season.
Check in the athletic office to see if your paperwork is current.
DON'T WAIT UNTIL THE FIRST DAY OF PRACTICE TO GET YOUR PERMIT.
Permits must be obtained for each season.

ATHLETIC PARTICIPATION FEE: Each athlete is required to pay a $41 participation fee per season. The family maximum per season is $82. Please keep in mind there are three seasons each school year making the annual family maximum $246. If you qualify for the free or reduced lunch program, you may apply for a fees waiver. The fees waiver is separate from the free and reduced lunch application and must be submitted within 30 days.

*PHYSICALS*

Sports physicals are required every other year, beginning in 7th grade or the first year students compete in interscholastic athletics. All athletes MUST have a WIAA physical card or alternate year card on file in the athletic office BEFORE they will be allowed to participate in practice or tryouts. Check Family Access >Health Information >Physical to view the last physical date on file.

Sports physicals are offered by many local clinics throughout the summer. Contact individual clinics for dates and fees.
