Need to create an online account?

1.  Click "Register Now".
2.  Enter your personal details (name, address, dob, etc.).  Click "Next".
3.  Choose Option 2 and fill in your Social Security number.  Click "Next".
4.  Fill in your username, password, etc.  click "Next".
5.  Review the information for accuracy and, if satisfied, click "Submit".
6.  Click "I Agree" on the next page.
7.  Make your selections of how you would like to be contacted on the next page and then click "Finish".


Need a claim form for medical or vision?

1.  Sign into www.mycigna.com
2.  Click on "Forms"
3.  Select and download the form you need (Medical or Vision)

Need a copy of your card?

1.  Sign into www.mycigna.com.
2.  Click on "Profile".
3.  Under "ID Cards>Print or Request Medical ID Card" select "Print or Request"

On and after July 1st of each year, you should be able to see a copy of your insurance card and print it out for temporary use until your new card arrives.

Need to be reimbursed by Cigna because you have reached your maximum on the prescription coverage?

1.  Complete the attached Prescription Drug Claim Form in its entirety. Note that the OTHER Box is already checked and the Employer Box is also pre-filled.

2.  Attach copies of your prescription receipts to this form. Please note that you can attach as many prescription receipts as you want so long as all the information required on the form must be on the receipt (i.e., Rx Number, Quantity, Day Supply, Drug Name and Strength, NDC, Amount Paid, Pharmacy Name, Pharmacy NABP, and Pharmacy Address)

3.  Keep a Copy for your records.

4.  FAX the form and Receipts to 860-731-2983 and attention Tracy Rogers.

    (DO NOT mail to the address on the back of the attached form!)

The Flexible Savings Account claims period runs from September 1 though August 31.   Ceridian has a close-out date for submitting claims for the previous year.  All your claims (for expenditures before September 1st), must have been completed by November 30, in order to be paid by Ceridian.

INSURANCE NEWS for 2015-16

Insurance CARDS! First, please note that we ARE NOT changing insurance carriers this year! However, as we have had in the works for a couple years, we are shifting our coverage to the Eastern Connecticut Health and Medical Cooperative (ECHMC). This is a group of five municipal and school districts that have come together to work on containing health care costs. The good news is that the premium rates for next year will not increase. The only increase you will see is related to your individual contracted premium cost share percentage. 

HOWEVER - you will be receiving a NEW MEDICAL INSURANCE CARD. It should be arriving from CIGNA and it will have new numbers associated with the ECHMC. PLEASE DO NOT THROW THIS CARD OUT! Also, if for any reason, you have not received a card by June 30th, please contact our Business Office (860-664-6506) directly and we will make sure one is sent out immediately.
