Welcome to my school nurse page.

Hello, my name is Jennifer Peters RN, school nurse for Preschool-12th grade.  This is my 8th year at Clarinda Community School District. I am married with 2 boys, Roger and Baker.

  • If your student has a food "allergy" or intolerance and the school needs to supply a substitute at meals a Diet Modification Request needs to be filled out and signed by a doctor.  This request has to be done yearly.  This form is called DietModificationRequest.
  • If your student has a Food allergy that requires an Epi Pen a Food allergy form needs to be filled out and signed by your physician.  This form is called Food allergy action plan.
  • If your student has an allergy to anything that requires an Epi Pen a form needs to be filled out and signed by your doctor.  This form is called  Epi20Pen20plan.
  • Forms for prescription and Over the Counter medications is called Record of medication. This is for any medications to be given at school prescription or over the counter, including inhalers either given by staff or assisting a student with an inhaler. 
  • If your child has asthma and can administer their inhaler on their own, a form needs to be filled out and signed by your doctor.  This form is called Asthma.pdf
  • For general information for any student using an inhaler at school, the form is called Inhaler's at sch0ol.
  • For all students with asthma needing an asthma action plan for school, the form is called asthma_actplan. This form needs to be filled out and signed by your doctor.

Immunization Information 2017-18

Immunization Requirements for students entering Kindergarten and above:  
  • DTap- 5 doses required with one dose after age 4 if born on or after September 15, 2004.
  • Polio- 4 doses required with one dose after age 4, if born on or after September 15, 2004
  • MMR- 2 doses required with one dose on or after 12 months of age, and one dose between ages 4-6
  • Hepatitis B- 3 doses required
  • Varicella- 2 doses 
There was a new law starting in May 2015 stating that all students entering Kindergarten and 3rd grade have to have proof of an eye exam.  The eye exam can be done up to a year prior to enrollment and no later than 6 months after enrollment. All students entering 7th grade are required to have a Tdap (Tetanus with Diptheria and Pertussis) vaccine  and a Meningitis vaccine prior to entering 7th grade.  The Tdap vaccine can be given regardless of when their last Td (tetanus) shot was given.  This is a state requirement.

Immunizations recommended for students 11-12 years of age:
  • Tdap (Tetanus with Diptheria and Pertussis)- required for entry into 7th grade
  • Meningitis vaccine ( with a booster dose at 16 years of age)
  • Human Papilloma Virus (HPV)
  • Hepatitis A- set of 2
  • Varicella- if child has not had 2 doses or had the a case of chicken pox

Snack/ Treats:  Due to food allergies students may have the school our school policy is any snacks brought into school for parties or treats have to be pre-packaged and labeled with the ingredients.


NOTES:  

It's that time of year for yucky noses, cough and fevers.  Please remind your kids to wash their hands frequently or use hand sanitizer to help stop the spread of germs! 

Reminder: The student has to be fever free without use of fever reducing medication to attend school. The student also needs to be 24hrs. without vomiting or diarrhea and no medication prior to returning to school.   

Cough drops may be sent to school with a note from parents.  In elementary they will be kept either by the nurse or the teacher and given as needed. In the middle school students may carry their cough drops.
