Garrett's Law - This law mandates that schools provide parents and guardians with information about meningococcal meningitis and influenza and the vaccines that protect against these diseases.

Homebound Instruction - This program provides instructional services for students confined for four weeks or longer at home due to a medical or psychological treatment.

CHCCS Youth Risk Behavior Surveys - These Centers for Disease Control (CDC)-designed surveys are administered to Chapel Hill‐Carrboro City Schools (CHCCS) students in odd years to monitor health risk behaviors that contribute to death, disability, and social problems among our nation’s youth.     7th Grade Parents- Tdap / Meningococcal  Info for 2017-18 School Year

    Parents, please note that your rising seventh-grader will need to receive the required Tdap and Meningococcal vaccinations. It's one of the important things you'll need to check off the list before your child starts school this coming school year.
    Tdap and MCV are available at your family doctor’s office. If you need help finding health services, click here

    All New CHCCS Parents- Health Requirements for 2017-18 School Year

    Parents, please note that your student will need a complete record of required immunizations to attend school. If your student is new to NC Public Schools they will also need a completed Health Assessment If you need help finding health services, click here.
    Please note: For students who start on the first day of school, 2017, the 30th day is September 26th, please ensure all records are submitted to avoid your student being excluded on September 27th

  • Health & Immunization Requirements: Important Information for Parents
    Students are required to have an immunization record on file to attend school in NC. Additionally a Health Assessment is needed on entry to NC public schools for the first time.
    Parents have 30 days from their child's first day of school to provide this information. On the 31st day, students without a complete immunization recordand/or HA on file will be excluded until it is received by the school nurse. 

    For students who start on the first day of school, 2016, the 30th day is September 27th, please ensure all records are submitted to avoid your student being excluded on September 28th
