Requesting a College Transcript from MCC 


To request an academic transcript, the College requires written authorization from the student. The College does not take transcript requests over the phone. Requests will be honored as quickly as possible in order of receipt; however, during peak periods such as registration and end of term, you can expect some delay. Transcripts will not be issued for students who have unpaid financial obligations to the College. If requesting a transcript sent to more than one institution, you must submit a separate request for each.

  1. Print out the form MCC Transcript Request Form (pdf) – Note: Adobe Reader is required to open the form.
  2. Fill out the form and sign your “written signature” in pen or pencil.

GRMC Auxiliary Scholarship - Applicants must plan to pursue a degree in the healthcare field. Several scholarships may be given - there is $2000 available for these scholarships. GRMC Auxiliary Scholarship Application

Iowa American Legion Auxiliary Scholarship -  Applicants must be great grandchildren, grandchildren, or children of veterans.  Along with 3 letters of recommendation and an essay, students need the signature of the Unit or Post Officer, whose name is on the application.  American Legion Auxiliary Scholarship Application

Key Cooperative Scholarship - Will award one non-renewable $1000 scholarship Applicants must have a parent or guardian that is a Class A or Class B member of Key Coop, have a non-weighted cum GPA of 2.65 or better, plan to enroll in an accredited two or four year college, university or vocation-technical school with a major course of study in an agricultural or ag-business field, animal science, communications, information systems, accounting, auto/truck repair, carpentry or masonry construction.  Scholarship Guidelines  Application

Poweshiek County Retired School Personnel Teacher Scholarship - Applicants must be plan to become a teacher and be enrolled in a college with a Teacher Education program.  $500  Poweshiek County Retired School Personnel Education Scholarship

Wendy's High School Heisman Scholarship - Applicants must have a B average, participate in at least one school-sponsored sport.  Selection based upon - Academics, Athletics, Leadership. Applicants must list someone to review their applications - your school counselor can do this. Apply online @  

The Horatio Alger Scholarship  - Especially for applicants who have financial need and have exhibited integrity and perseverance in overcoming personal adversity.  Criteria is a commitment to pursue and complete a bachelor's degree. You may apply onlline  at   

The Jack Kent Cooke Foundation Scholarship -  Designed to encourage and support outstanding students who work hard and have financial need (30-40 awarded nationally).  For more information, check out this link: - Phase 1 of the application.

The Better Business Bureau Foundation - Students selected must personify high character and ethical behavior in their leadership and contributions to their communities. Five $2500 scholarships will be awarded to high school seniors in Iowa.  BGM can submit one application.   Students will write an essay about a situation where they made an ethical decision.   - One student will be selected by BGM BBB app

The Prudential Spirit of Community Award -  Recognizes students in grades 5-12 who have played a leadership role in a community service project within the last 12 months. Schools will choose one nominee to continue in the competition.  Students need to turn in applications by 10/25/17 to the Guidance Office in order to be considered for this award.  Applications are available at:    

Hagan Scholarship Foundation Scholarship -  Applicants must be seniors from a public high school in Iowa and in a county having fewer than 50,000 residents.  Scholarships will vary up to $5,000 per semester.  Each scholarship is renewable for up to seven additional semesters if you maintain a 3.0 gpa in college.  Application can be found at: 

The Robert Smiley Scholarship is offered by the Iowa Girls' High School Athletic Union and the Smiley Family in honor of the longtime IGHSAU Associate Executive Secretary who passed away in 2003.  Applicants must be female and letter in at least one varsity sport each year of HS.  They must also have a minimun GPA of 2.5.  Applicants experiencing hardship (financial or other) will receive special consideration.  BGM can submit one applicaiton.  A scholarship committee will select the BGM nominee.  Applications can be found at the following website in the left-hand column:   

 The Elks National Foundation  - Most Valuable Student Scholarship -   Any high school senior who has high academics, is active in their schools and communities, and recognized as leaders by their peers may apply.  To apply go to and click on the MVS application link.    (to the Elks Lodge closest to your home). 

AXA Achievement Scholarship & AXA Achievement Communiity Scholarship - Are you active in your community?  Have you led or initiated a project that benefits others? The Iowa Ready Mixed Concrete Association (IRMCA) Scholarship - Applicants need to be entering their first year of an Iowa Community College or Tech School building trades program that provides training in concrete technology and practice.  The amount of $2,500+ will be given each year of a 2 year program.  Online application:

National Turkey Federation Scholarship - Seniors must have a 3.0, a current hunting license and a current National Wild Turkey Federation membership.  Online application:  
 Profile in Courage Essay Contest - The John F. Kennedy Library Foundation invites U.S. high school students to write an essay on an act of political courage by a U.S. elected official who served during or after 1956.  First place prize worth over $10,000.  Second place - $1,000. Burger King McLamore Foundation Scholarship - All HS seniors with a GPA of 2.5 are eligible to receive up to $50,000 in scholarship awards.  Go to:    

The United States JCI Senate Foundation Scholarship - Available to high school seniors who live in a state with an active JCI Senate program.  Applications are in the Counseling Office and at:   

The Ronald McDonald House Charities Scholarship Program -  Seeks to invest in students who are exemplary role models in their communities and have the initiative to fulfill their educational goals.  Visit  for complete program details  

The Elks National Foundation Legacy Scholarship -  Applicants must be children or grandchildren of dues-paying Elks.  Applicants are judged on the core values of the Elks, knowledge, charity, community service, and integrity. Applications online at  

Christina Hixson Opportunity Award.  - Applicants must attend Iowa State University or Creighton University.  One recipient will be chosen from each of Iowa's counties. The award is a half-tuition scholarship for all four years.   Students must have faced challenges of personal and/or financial hardship.  Applicants must apply to the college and submit the Hixson application by Feb. 1.  The FAFSA must be filed by March 1.   Go to:

Iowa 4-H Foundation Scholarships -  Scholarships range from $500 - $3500. Applicants must be former or current 4-H members. 4-H Scholarships 

The Grand Lodge of Iowa Scholarship -  Approximately sixty $2000 scholarships are offered to Iowa high school seniors who will continue their education at a two or four year college /university.  Service to school and community with special emphasis upon leadership roles are important.  An applicant's academic record, communication skills, and need for financial assistance are also carefully considered. Iowa Scholarship for the Arts -  awarded to graduating seniors who have shown outstanding ability in one or more of the following areas:  music, dance, literature, theatre, traditional arts, or visual arts.  Applicants must be accepted as a full-time undergraduate student at a fully accredited Iowa college or university.   Detailed guidelines and application requirements can be found by visiting the website at:  (look under Programs)  $1500  

 The Hy-Vee Foundation Scholarship Program  - Offers forty $1000 scholarships to high school seniors who have worked for Hy-Vee or subsidiary since Sept. 15, 2013. Sons/daughters of full or part-time Hy-Vee employees (six months employment as of Sept. 15, 2013) are also eligible.  Student must have an ACT score. World Class Scholarship Program - Mediacom Communications is offering sixty $1000  scholarships to seniors who live in the service area.  Applicants need not be customers. Applications can be downloaded at:  

National Co-op Scholarship Program  Given to graduating high school seniors who will attend one of the eleven college and universities in the national cooperative education group and have a 3.5 or better gpa.  Online application:   

Buick Achievers Scholarship - Intended for applicants with financial need who put forth the effort to have success both in and out of the classroom. 100 scholarships each up to $25,000 per year.   For students who plan to major in a field of study that focuses on engineering, technology, design or business, with an interest in the automotive industry.  Go to:  for more information.  

Iowa Soybean Association Ag Scholarship - Applicants must attend a 2 or 4 year college and study an in Ag-related field.  One recipient in each of the nine crop districts in Iowa will receive $1,500. From the nine district recipients, one statewide winner will be selected and will receive an additional $1,500.  Iowa Soybean Ag Scholarship application  

The Matthew Shepard Scholarship  -  Awarded to openly LGBTQ Iowa high school students. Applicants must have a 3.0 GPA and an ACT Composite score of 22.  Iowa's Gold Matthew Shepard Scholarship is an award of up to $40,000 over four years if you attend The University of Iowa, Iowa State, or the University of Northern Iowa.  Iowa's Silver Matthew Shepard Scholarship is an award of up to $9,000 over 4 years at any college or university in the U.S.  The application is available at:  

Iowa Farm Bureau Federation Scholarship - Applicants must be son/daughter of a Farm Bureau member, have a 2.5 GPA, and pursue a degree in an ag-related field.  Go to:   Select:  My Membership.  

Iowa Automobile Dealers Foundations Scholarship - Will award up to 15 scholarships of $2000 to students who plan to pursue training in automotive-related studies, accounting, or business administration.  Must have a copy of your Estimated Family Contribution from the FAFSA.  Iowa Automobile Dealers Scholarship  

MCEC Technical Scholarships - Awarded by The Grand Lodge of Iowa's Masonic Charitable Educational Corporation.  Scholarships will be awarded to Iowa high school seniors who plan to enroll in a mechanical or trade course of study at an accredited Iowa Community College.  An applicant's academic record, communication skills, financial need and plans for the future are considered carefully. ACACIA Leadership Scholarship -  Available for seniors males who will attend Iowa State University. The scholarship is awarded by the local Corporate Board of Alumni of ACACIA Fraternity. Maximum value - up to $1000.  Preference is given to students associated with the fraternity.  ACACIA of ISU Scholarship 

The Dale Schroeder Memorial Scholarship - Awarded to seniors who reside in a community with a population up to 10,000.  Students must have a GPA of at least 2.8 and attend Iowa State, University of Iowa or Northern Iowa. Applicaiton is online:  

The Herbert Hoover Uncommon Student Award -  $5,000 scholarship program for Iowa high school JUNIORS. The program annually identifies and honors up to 15 Iowa high school juniors who propose and then accomplish a project of their own choosing and design. Grades, test scores, essays and financial need are not evaluated.  For more info go to:   

 The Bright Scholars of Iowa Award  - is a need-based grant designed to help ensure that selected outstanding Iowa high school seniors have the financial means to complete a two-year or four-year college education without debt.  Scholarships will be awarded to students who plan to attend UNI, ISU, IOWA or DMACC, Southwest Iowa CC, Indian Hills CC.  Applicants must be a high achieving student, and qualify for a Pell grant as determined by filing the FAFSA  The amount of the award will equal the amount of the first year of attendance.  Go to:  Deadline varies by institution.

Iowa Family Farms Scholarship - The Gardner family (Iowa Family Farms) will provide two high school students with scholarships of $500.  One recipient must be from Newton HS.  The other may be from Poweshiek, Jasper, Marion, or Mahaska county.  Your GPA must be 2.8 or higher.  Download the application at:

Cosmetology Scholarship - Des Moines area Great Clips salon owners offer three $300 scholarships for individuals interested in pursuing a career in cosmetology to high school seniors.  Scholarships can be used at any accredited cosmetology or barbering school in the Des Moines area.  Cosmetology Scholarship application
