Google Forms

To have students respond to a FORM, you will need to place it on your Google Site (webpage) where they can access OR email the form to students. 

1. Create your form in Google Drive. There is a template for the Stronge Student Survey Grades 6-8 in the template gallery. (Click CREATE > From template...  and select STRONGE: Grade 6-8 Student Survey (old).) Your copy will be called Copy of Stronge... and be saved in My Drive in Google Drive.

2. Open your Google Site (webpage) to the location where you want to place the survey and click the EDIT (pencil) icon. (Click the Google App icon near your username and select Sites.) Decide where on the page you want the survey and make sure the blinking cursor (insertion point) is at that location. 

3. Click INSERT > DRIVE > Form and click the box in front of the form title (STRONGE: Grade 6-8 Student Survey (old)).

4. Click SELECT on the bottom of the screen after clicking the checkbox in front of your survey.

5. Remove the width in the dialogue box (field will be empty) and click SAVE at the bottom of the window.

6. Click SAVE at the top right of your webpage.

The form will now appear on your webpage and you can direct students to the location to complete the form.

TO SEE RESULTS:

1. Open Google Drive and go to My Drive.

2. Open your survey and choose  FORM > Show Summary.

You can also LINK the form by copying the address for the live form and applying it to text on a webpage.

TO TURN OFF SURVEY:

1. Open your survey and choose FORM > Accepting Responses. This will remove the checkmark that was in front of Accepting Responses and the survey will now display an error message.

2. To activate the survey again, open your survey and choose FORM > Accepting Responses. This will return a checkmark in front of Accepting Responses and the survey will now display on your webpage.
