Testing Center Guidelines
The following guidelines have been established by the Testing Centers “to ensure test integrity and adequate space for testing”. The hours of operation are also given there.
Before initial use of the Testing Center, faculty should contact a campus Testing Center supervisor to receive an orientation to Center services. All Distance Learning faculty should contact the Riverside Testing Center supervisor. If you are using tests written by another instructor, please provide written permission from the author and submit your own transmittal forms. Please include a copy of the Student Guide for Use of ACC Testing Centers in your syllabi. For security reasons, instructors may not call Testing Centers to amend test instructions.
Initial tests for classroom credit courses are limited to:
Distance Learning courses delivered via telecommunications or print-based methods.
certain approved classroom courses (none of which are in our area)
Re-tests and make-up tests (alternate versions) are limited to one per student per exam for all courses. For all courses, no more than eight (8) tests with four (4) versions or forms (A, B, C, D) each may be placed in the Testing Center. Students may not test and retest on the same day and must bring the yellow student copy of the Student Test Request Form in order to retest. Challenge exams, assessment tests, and tests to remove grades of Incomplete (students must be provided a Report of Incomplete Grade Form to present to the Testing Center) are authorized; timed tests for academic courses are not administered. All exams will be collected from students thirty (30) minutes after the stated closing time of the Testing Center. Lecture class exams cannot be graded in the Testing Center. Group testing is not allowed.
Please make arrangements with your campus Office for Students with Disabilities for students requiring individual assistance or special testing needs. For security reasons, Testing Center policy does not allow tests to be taken out of the Testing Center for student use, therefore, tests cannot be administered if the required individual assistance or special needs cannot be accommodated in the Testing Center.
Tests are not accepted during the last week of each semester.
Tests must be submitted to the Testing Center (in a sealed envelope if via intercampus mail) at least two working days before students are expected to start taking them.
Only instructors may submit tests.
If a multiple-choice test is to be scored by the Testing Center, the test must be submitted with a Scantron answer key on the correct Scantron form.
A Transmittal Form must be submitted for each test each semester reflecting the new synonym/section number. Distance Learning exams may remain active for up to one year from date submitted. Test for Distance Learning courses must be placed in all Testing Centers. On campus (lecture) courses can remain active in the Testing Center for a maximum of one semester. If deadlines and instructions stated on the Transmittal Form are the same for every test in a series for a particular course, only one Transmittal Form for the series is required for each Testing Center. All test deadlines are at the end of the Testing Center’s workday.
Tests not adhering to the following format will be returned for correction:
Test copies must be legible and printed on 8 ” x 11” paper.
The following information must be printed on the first page or on a cover sheet attached to each copy of the test. This information must also be listed on any additional materials provided, i.e. periodic tables:
Instructor’s Name............................................DAVID DOE
Course Abbreviation and Number................HIST 1301
Test Number (Arabic Number Only).............TEST 1
Version or Form (if applicable A,B,C,D).......B
Supplemental Materials Allowed..................SCANTRON
Or indicated - No Supplemental Materials
Students must present valid I.D. (photo I.D. and ACC student I.D.) for admission to the Testing Centers. An instructor can escort students without a valid photo I.D. at the time they take the test.
You may provide students a permission slip to authorize testing but it will not be in lieu of a Student Test Request Form. Permission slips will be returned attached to the completed.
Students are allowed only those supplemental materials listed by you on the Transmittal Form. Approved items such as English dictionaries (non-electronic), Scantron answer sheets, and all types of paper are provided by the Testing Centers. Open book exams are not permitted. All other items (pagers, cell phone, laptops, purses/wallets, food or tobacco items, calculator/pencil cases, etc.) are considered unauthorized and students will be asked to leave the Testing Center and subjected to scholastic dishonesty procedures if violated. An instructor can escort students without a valid photo I.D. at the time they take the test.
The following scholastic dishonesty procedures will be followed for students suspected of and/or caught cheating (including the use of unauthorized materials):
The student will be referred to the appropriate administrator.
A Notification of Alleged Scholastic Dishonesty Form will be forwarded to the student’s instructor and the Student Discipline Form will be forwarded to both the instructor and Campus Dean of Students (with test attached).
The instructor and Campus Dean of Students will make a determination whether scholastic dishonesty has occurred and a notice of final ruling and disposition will be issued, according to the Student Discipline Procedures.
A confirmed incident will result in the student being barred from Testing Centers for the current semester for that course. Testing Centers will maintain an updated list of barred students.
Tests taken at the instructor’s home campus will be available for pick-up in the Testing Center (unless otherwise indicated on the Transmittal Form). Tests taken at campuses other than the home campus will be mailed to the instructor’s home campus the day after the exam is completed. The white Student Test Request Form will be mailed to all instructors on a weekly basis.
