A Guide for Volunteers

 The following information is requested of a Volunteer in our School District. Please complete the following forms and return the paperwork to your site secretary. The secretary will need to see your Driver's License to verify the information on the paperwork you have filled out.

1.  Volunteer Application/Agreement Form - Please complete both sides of the form.  If you have students        at different school sites, you will need to fill one out for each of the schools so each Principal can sign off on      it.   
2. TB Risk Assessment/TB clearance ~  (TB clearances are good for 4 years from the date of clearance) If you 
     have had one in the last 4 years, we will just need a copy of it.  
** We now accept TB Risk Assessment's. MED Post in Paso Robles (805-226-4222, the Health clinics in San Luis      Obispo and Paso Robles are offices that offer the Assessment.) This consists of a series of 5 questions. If          you are not considered of "high risk" after answering the 5 questions, the doctor's office will clear you. 
     YOU MUST SPECIFICALLY ASK FOR THE "TB RISK ASSESSMENT" otherwise, they will automatically give you
     the TB shot.
3. Fingerprint Clearance -  if you plan to chaperone on any field trip, you MUST have Fingerprint 
     Clearance through the San Luis Obispo County Office of Education. The cost is $77.00. It is a ONE 
     TIME fee and you are on the Atascadero Unified School District's fingerprint list for LIFE!

     FINGERPRINT FORM CAN BE EMAILED to YOU to save you a trip to the office.
