A Guide for Volunteers

The following information is requested of a Volunteer in our School District. Please complete the following forms (1 and 3 below) and RETURN the paperwork to your site secretary. The secretary will need to see your Driver's License to verify the information on the paperwork you have filled out. This is also our Megan's Law check.
1.  Volunteer Application/Agreement Form ~ Please complete a Volunteer Application/Agreement form (2           pages) for each school site you wish to volunteer.        You may also pick it up at the school site.  The form is valid for one school year only.  For the 2018-2019
       school year, turn in your new 'paper' form to the site secretaries WHEN THEY RETURN IN AUGUST.
2.  Online Pre-Volunteer Application ~ If you completed this during the 2017-2018 school year, you do           not have to do this again. If you did not complete this in the 2017-2018 school year, please complete         the ONLINE portion of the application.  Click on the link provided below. This places you on the Volunteer         database.  Once approved, you will be able to login under Volunteers, when going on campus.  This also             allows you to receive emails on the process of your application.

3. TB clearance/TB Risk Assessment ~  Please attach a copy of your current TB clearance with your                        Volunteer Application/Agreement form. (TB clearances are good for 4 years from the date of the                          clearance). If  you have had one in the last 4 years, we will just need a copy of it.  
** As an option, we also accept TB Risk Assessment's. This consists of a series of 5 questions. If you are not           considered of "high risk" after answering the 5 questions, the doctor's office will clear you immediately.             MED Post in Atascadero, (805-703-2501), 7330 El Camino Real OR Paso Robles (805-226-4222), 500 1st                 Street, offers this assessment. There is a $20.00 fee. When calling to make an appointment,  YOU MUST             SPECIFICALLY ASK FOR THE "TB RISK ASSESSMENT for Atascadero Unified School District", otherwise            they will automatically give you the TB shot and the fee will cost more. If you ARE considered of High Risk,          they will give you the shot right then and there and the fee is still $20.00.  The Health Clinics in Paso Robles        and San Luis Obispo also offer the TB Risk Assessment.
4. Fingerprint Clearance ~ If you plan to Chaperone on any field trip, you MUST have Fingerprint 
     Clearance through the San Luis Obispo County Office of Education and it must be specifically for the       School Districts in the County. Police Department fingerprints are not accepted.  Make sure to start           on this process very early as prints can take 2 days to several weeks to clear. The cost is $63.00. It is a                 ONE TIME fee and you are on the Atascadero Unified School District's fingerprint list for LIFE! ***Please             CALL Barbara Weaver at 805-462-4215 for information regarding this process and for the                         required form you need to take with you to your appointment. The FORM CAN BE EMAILED TO                           YOU, saving you a trip to the office.
