HEALTH SERVICES INFORMATION


    
Dr. David Pangburn, of Alewife Brook Community Pediatrics, Arlington's School Physician, provides consultation and guidance to the school nurses throughout the year.
 
Health services provided within the Comprehensive School Health Program include:
  •      Monitoring compliance with state immunization guidelines
  •      Illness and Injury assessment and care
  •      First aid assessment and treatment
  •      Medication administration and monitoring
  •      Individualized health care planning and case management for students with special health care needs
  •      Referral / Counseling / Guidance
  •      Health Screenings:
                       Vision (K-5, 7,10)
                       Hearing (K-3, 7,10)
                       Postural Screening (5-9)
                       Heights and Weights (1, 4)
                       Body Mass Index (1, 4, 7, 10)
  •      Communication with students, parents, the school community and health care providers
  •      Access to health care services and insurance
  •      Emergency preparedness and response
  •      Promotion of healthy life-style choices.
 
Immunizations Needed for School
  • Immunizations Required;  5 DTP/DTAP
  •                                                                   1 Tdap ( grades 7-12)                
  •                                                    4 Polio (OPV or IPV)
  •                                 2 MMR
  •                                  3 Hep B
  •                                                                              2 Varicella (or certified history of Chicken Pox)

Please note that immunizations need to be current from your primary care provider.  This is a state mandate.  Only religious or medically based exemptions are allowed for students who are not fully immunized to be in school.  

 

 Registration Requirements to Start School

Please see the following checklist to register your child for school.  
    
 Exam by a Licensed Medical Provider
(within the last 12 months or 30 days after school admittance)

 Immunizations Required;

 5 DTP/DTAP
1 Tdap ( grades 7-12)
4 Polio
2 MMR
3 Hep B
2 Varicella (or certified history of Chicken Pox)

________ Lead Screening (kindergarten only)
________ APS health history form completed, done online with registration
________ APS Emergency Form, done online with registration
________ Appointment with your school nurse if your child has any special health care needs including a chronic illness or emergency medications (Epi-Pen, Diastat) that will be needed at school.
