Student Placement Form and Instructions


Dear Parent/Guardian:

The primary focus of Thomas Lake Elementary School is to provide a positive educational experience for all students. Since the dynamics of specific groups are vital when attempting to maximize learning, the grouping of students into classrooms is extremely important.

In the spring of each year, teams of grade level teachers carefully consider the academic and social needs of each student to create class lists for the upcoming year. Tremendous effort is given to designing compatible, well-balanced learning groups. The following factors are used when making placement decisions and will ultimately result in a system that is not only educationally sound, but equitable as well.

  • Learning style(s) of student

  • Specific behavioral needs

  • Balance of achievement levels

  • Placement of students who work well together

  • Balance of boys and girls in each class

  • Balance class sizes

  • Parental input based on specific educational needs

  • Other factors as identified

    The process for developing class lists is as follows:

  1. The parent or guardian must complete the Student Placement Form. Please remember that requests will only

    be accepted in the form of teacher characteristics and/or special education concerns. Requests that name a specific teacher will not be honored; the forms will be discarded. Requests to place two students together will only be considered if both sets of parents sign the placement form. Requests to separate students will be prioritized by staff and may not be honored. Requests regarding placement in a combination class or split teaching assignment will not be honored.

  2. Forms for Grades K-5 students must be returned to the school office no later than Friday, March 17. Requests turned in after the deadline will be marked LATE and given a very low priority. If requests arrive after class lists are completed, they will be discarded.

  3. The teachers at each grade level will meet and design a class list considering all the criteria. The final responsibility for student placement rests with school personnel.

  4. Families will be notified during the summer with the name of their child’s teacher and directions to register for August assessments.

Please be assured that the Thomas Lake staff will review each child’s needs individually and will carefully select the classroom in which we believe your child will learn the best.
