Mentor teachers are required to fill out three forms during the first semester. Form B is the Professional Characteristics Feedback form (see overview here). Interns are assessed on professional characteristics by faculty instructors, supervisors, and mentors. These forms are confidential, but if problems are indicated, follow-up occurs with the intern and faculty or the department chair. Interns who received repeated concerns based on this form may be placed on a plan of improvement.
Form J is an observation form based on classroom experiences. Mentors observe interns and fill out a minimum of three of these during the fall semester. They are turned in to the Secondary Program at the end of the semester.
Form G is the mid-year assessment of the 8 Alaska Teacher Standards. This form documents progress made toward achieving those standards and is completed in conference with the intern and the university supervisor. The second semester counterpart, form H, is the final assessment of that requirement. Form G documents the beginning stages of an intern's journey toward licensure.
