Madras High School-Based Health Center
Serving patients, 0-18 years of age (19 if enrolled in school), the school-based health center is a partnership between Mosaic Medical, Jefferson County Public Health, and Jefferson County School District 509-J.  The SBHC is located in the modular building behind the tennis courts on the MHS campus.

Monday:        8AM - 5PM
Tuesday:       8AM - 5PM
Wednesday:  8AM -12PM
Thursday:      8AM - 5PM
Friday:           8AM - 5PM
Saturday:       CLOSED
Sunday:         CLOSED

Requirements for School Registration
State law (HB 2972 (2015)) requires a child who is 7 years of age or younger to have a dental screening before entering school for the first time. 

The following form is required to have on file at your child's school:
