Attention Current 6th Grade Students
Vaccine requirements for 7th grade include a TDaP (Tetanus, Diphtheria, and Pertussis) booster!  Contact your healthcare provider, the Jefferson County Health Department, or the School Based Health Center at Madras High School for this vaccine.

The Madras High School School-Based Health Center is open! Serving patients, 0-18 years of age (19 if enrolled in school), the school-based health center is a partnership between Mosaic Medical, Jefferson County Public Health, and Jefferson County School District 509-J.

Requirements for School Registration
State law (HB 2972 (2015)) requires a child who is 7 years of age or younger to have a dental screening before entering school for the first time. 

The following form is required to have on file at your child's school:

    Dental Screening Certificate/Opt Out Form English | Spanish
