3/1/09 - Eventing entries due to Perky- 2009 Medical Release Form per competitor (blank attached below)
- 2009 Activity Release Form per competitor (blank attached below)
- 2009 Competitor Information Form per competitor (blank attached below)
- Eventing Affidavit per competitor (if applicable, blank attached below)
- Copy of current negative Coggins per horse
- Chaperone Form if willing to serve (blank attached below)
- Coaching Form if applicable
- Check payable to Wild at Heart - $165 per competitor riding ($65 Horse Manager) ($20 per Chaperone) [late entries accepted until 3/10 with $25 late fee] These fees include Fri & Sat dinners and Sat & Sun lunches for competitors and chaperones. All others can purchase meals in advance.
