You will also find the parent/guardian signature sheet that must be

signed and returned to the Wellness teacher.

In addition, we have provided a copy of the medical assignment for those

students who are unable to participate in PE class due to a medical reason. 

These assignments (in conjunction with unit quizzes) will determine the

unit grade for these students. It is the students responsibility to ensure

these assignments are completed and submitted, as well as each unit quiz.
