1) If you are a new patient, please print the first two documents (patient data sheet and acknowlegement of receipt of privacy practices), fill them out and bring them with you to your visit.  You are also supposed to read the 3rd form (the privacy document), but you don't need to print it.
2) If you have records at another doctor or facility that you would like to have sent to me, please print the 4th document (release of records form), fill it out and sign it and send it to the previous doctor.
3) If you have medicare, please print the 5th document and bring it with you to your visit.
