H1N1 Clinic for Priority Groups - December 12

The City of Westfield, in conjunction with the Hamilton County Health Department and Westfield Washington Schools will conduct an H1N1 clinic for priority groups.  The schedule for the clinic is as follows:

Date:  Saturday, December 12, 2009
Time:  10:00 a.m. to 2:00 p.m.
Location:  Washington Woods Elementary, 17950 Grassy Branch Road, Westfield, IN.

The clinic will be for those defined as a priority per the CDC:

Pregnant women
Individuals 6 months to 24 years of age
Individuals with chronic health problems that are 25-64 years of age
Healthcare and EMS
Guardians of children younger than 6 months

Only those in the CDC priority groups as listed above should attend this clinic.

To learn more about the clinic, the H1N1 vaccine and to obtain vaccine consents forms citizens are encouraged to visit the Westfield READY website at http://www.westfieldready.org.  Citizens are further encouraged to print the consent forms from the website and complete the forms prior to arriving at the clinic.  Citizens must understand a completed form does not guarantee the receipt of a vaccine.

The times listed for this clinic are while supplies last.  Approximately 500 doses will be administered.  When this supply is exhausted, the clinic will close.
