Audition Application

Use this form to request an admission/scholarship audition time at the USA Department of Music. This form cannot be completed without a valid student ID number (J-Number), as no student may audition for the Department until they have been fully admitted to the University. Therefore, do not request an audition date until you have been fully admitted and have this ID number. If you cannot be present on the audition dates listed, or if the dates listed are in the past, then please check the box asking to arrange a special audition time. When you are finished, click the "Submit" button. We will contact you within two weeks of the audition date to confirm your assigned audition time and appearance.
