Interim Committee members (chair to be the likely mentor)
- Sends
Interim Committee members notice they are on the committee, student photo
and personal statement (from application)
- Updates
the master record with the interim committee members
- Sends
memo to the student with a list of committee members and asks student to
schedule a meeting with this committee

academic background
- Discuss

Form I (sign and forward to Graduate Chair)
Graduate Chair
- Endorse
Form I and forwards to the Graduate Division
- Record
date in master records
- Establish
a record of the proficiencies that are outstanding
