You must have your registration # to complete this form. The registration # is the number on the top right of the Registration Card that you receive from the CSB Cashier when you pay your Bond. If you are transferring a Bond from a previous sport, you should use the registration # that was on the original Registration Card. You can call the office at 831-2539 and get the number from the staff if available. (Any forms that are submitted without a valid Registration # will be removed from the list.)
Rosters will need to be filled out. All players must come by the IM Office (134 CSB) and sign the roster before they play. This can be done anytime before the player plays in a game.
Someone must represent your team at the Captains Meeting. If a team does not attend the meeting it will lose $25 of the bond and may not be scheduled. At the meeting, teams will select their playing times. This selection will be done based on order of registration. The order of registration will be created by the time that the registration form is submitted online. (The form is time stamped.) (if there are 50 teams that all register at 12noon, you could submit the form at 12noon and still be the 50th team.)
On the registration form, you will select the level you wish to play. Men's, Coed, or Women's and Blue (highest), Gold (middle), or White (lower).
At the captains meeting, once a team selects a spot in a league, the team is responsible for those games. A team may default ONE game (by filling out a default form in the IM Office 24 hours prior to the game.) But times will not be switched and teams may not withdraw from the league after a time is picked. If a team misses a game they will forfeit and lose $25 of their bond. Registration forms will be kept open for late teams. Teams that miss the deadline to register may still fill out the registration form. They will be put on a wait-list and if there are open spots in the leagues or teams are dropped from the leagues for forfeits a wait-listed team may be placed into the league.
One Night Swim Meet Individual There is no bond needed for this event. The Swim Meet will be held on Wednesday 12/2/09. Participants should arrive at the CSB pool (on deck) no later than 7:15pm.
